By submitting this Form I authorize Madrigrano, Aiello and Santarelli, LLC to file the BOI Report and certify that all information is current, true and

correct. Further, I acknowledge, agree, represent and warrant as follows: (1) that Madrigrano, Aiello and Santarelli, LLC has no obligation to file the

BOI Report until payment has been successfully submitted and I have provided Madrigrano, Aiello and Santarelli, LLC with any and all necessary

documents required to be submitted; (2) that before submitting this form I have communicated with Madrigrano, Aiello and Santarelli, LLC if there

were any questions that I or fellow owners/controlers had; (3) that I, the Submitter am authorized to submit this form on behalf of the Entity requesting

assistance; (4) that changes to the information above needs to be corrected with an additional filing; (5) that Madrigrano, Aiello & Santarelli, LLC has
